What can I expect at my appointment?
During your initial visit, besides paperwork, the therapist will also discuss your medical history, current medical problems/pain, your physical limitations, your goals for therapy, medications you're taking, and procedures you've had relating to health.
What do I need to bring with me?
For your appointment, you will need to bring your completed paperwork, list of current medications, your driver’s license/ID, and insurance cards. Be prepared to answer questions about your medical history. We'll also make an initial evaluation.
Also, be sure to bring your physical therapy referral from your physician or Worker's Comp claim information.
Do you take my insurance?
If your insurance company is not listed, we can verify your benefits with your particular insurance company. We'll be glad to work with you to make sure you know the exact benefits you receive from your insurance. We accept a variety of insurances such as: BlueCross BlueShield, MS, Humana, Health Link, Acclaim, Medicare, United Healthcare, Workman's Comp., Cigna, Tricare, Champ VA, Aetna, and Self Pay.
